  *According to the instructions of COCIS (Cardiological Organizational Committee for Sports Fitness) for athletes aged at least 35 the maximum exercise ECG must be obtained.
  You must have: sports clothing (gym shoes and track suit/shorts); men must have a shaved chest

• Documentation to submit for the appointment
  1. Urine sample with sediment results, dated no later than 30 days prior to the appointment
  2. ID document (minors must be accompanied by a parent, with an ID document)
  3. Tax ID number
  4. Green sports records (if possessed)

Attention

As per the latest provisions of the Italian Sports Medical Federation, anyone who has contracted the SARS-COV2 virus, ascertained with a molecular swab, must undergo a new “return to play” sports medical examination.

It is emphasized that whoever is in possession of a valid sports medical examination and has contracted the SARS-COV2 virus, even if asymptomatic, must undergo a medical examination again, as per previous indications to return to competitive sports.
