Certificates for CUSB members - Bologna office

👉 What You Need to Know:

Non-competitive medical examination for CUSB membership ➡️ The certificate will not be issued and is only valid for participating in CUSB activities.

⚠️ You can request the issuance of a medical certificate that can be used for both CUSB membership and external activities, but the examination will cost more.
⚠️ If you already have an electrocardiogram, you must bring the original to show the doctor and a photocopy of the certificate to leave with the doctor. If you have additional cardiological documentation, it is always advisable to bring it with you.
Please Note: The ECG is only valid if performed after September 1, 2023, and must be accompanied by a report ➡️ Athletes over 60 years old or those with specific medical conditions must have an ECG every year.

• Period for Examinations:
  - Front Office → September to May
  - CUS Record Facility → September to November
• Costs:
  - Medical examination ➡️ €20 ⚠️ (ECG already in possession)
  - Medical examination + electrocardiogram ➡️ €30
  - Medical examination + electrocardiogram + issuance of certificate valid for non-CUSB activities ➡️ €50

⚠️ If you have already made a reservation through the website, you can no longer modify it or make a new reservation:

➡️ Minor athletes must be accompanied by a parent or legal guardian
